    Y Supportive Housing is a community solution to help eliminate homelessness by providing Hope, Dignity, and Support in the form of permanent supportive housing. This Housing First Model helps people who are homeless (or who are at-risk of homelessness) move into stable and secure permanent housing supplemented by supportive services. Case management, transportation assistance, food/clothing/hygiene pantries, rental assistance, and other services help people meet their basic needs and reach their life goals. The campus provides a place to call home for 140 men and women. The constant wait list demonstrates the need for affordable housing in the community.


    Y Supportive Housing is part of the YMCA of Greater Des Moines Association, whose mission is to put Christian principles into practice through programs that build healthy spirit, mind and body for all.


    Reporting to the Supportive Services Program Director, this position is responsible for providing case management services to an assigned case load of residents.  The Case Manager will assist in delivering supportive services programming, including internal community groups and education sessions and will also work with external agencies, building relationships and resources to better support our residents.

      1. Manage supportive services grants, including requirements, compliance and reporting.
      2. Prepares intake information and conducts intake interviews on participants.
      3. Provide counseling and case management services as assigned and maintain effective and efficient files and documentation as required.
      4. Follow policies and procedures of the supportive services programs and YMCA.
      5. Develop, plan, manage and deliver resident groups, trainings and courses.
      6. Coordinate external resources in the delivery of or referral to supportive services.
      7. Foster and encourage an environment of community through relationship building.
      8. Provide and collaborate within a positive environment in which residents are encouraged to be actively engaged in the internal community and the change process.




    1. Develop and maintain collaborative relationships with community organizations.
    2. Serve as a YMCA representative, both within the organization and outside the organization.
    3. Demonstrate and support the objectives of the organization as embodied in the mission statement and values.




    1. Assist in organization’s financial development efforts, including the annual fund raising campaign.
    2. Plan, prepare and organize special events as required.
    3. Serve on organization and community committees and teams as required.



    1. Bachelor’s Degree in Social Work or related field or equivalent
    2. American Red Cross CPR-PR/AED/First Aid Certifications or equivalent certifications approved by the YMCA
    3. American Red Cross Blood Borne Pathogens Training or equivalent training approved by the YMCA
    4. Y Orientation Session
    5. Y Emergency Procedures Training
    6. Demonstrated computer skills in a Microsoft Windows environment
    7. State of Iowa License in Social Work, Mental Health, or Substance Abuse preferred, but not required.
    8. Masters Degree in Social Work or related field preferred, but not required
